Humanity has lived with nuclear weapons for more than 80 years. The extraordinary fact is not simply that they have never been used in war since Hiroshima and Nagasaki. It is how many times governments, soldiers, computers and warning systems have brought the world frighteningly close to using them again.
Some crises involved presidents consciously weighing military actions that could have escalated into nuclear exchange. Others were stranger and arguably more disturbing: a malfunctioning computer, sunlight reflected from clouds, a scientific rocket mistaken for an incoming missile, or a military exercise interpreted as preparation for a real attack.
The danger has not disappeared. At the beginning of 2026, the world's nine nuclear-armed states possessed an estimated 12,187 nuclear weapons. Around 4,012 were deployed with operational forces and roughly 2,100–2,200 were maintained on ballistic missiles at high operational alert. Russia and the United States still dominate the global stockpile, while China is expanding rapidly and India, Pakistan and North Korea continue developing their forces.
So which incidents really brought humanity closest to nuclear war?
There can be no mathematically definitive ranking because historians cannot know what leaders would have done in alternative timelines. But judged by proximity to actual nuclear use, military readiness, time pressure, misunderstanding and the likelihood that one further escalation could have triggered retaliation, these stand above the rest.
1. The Cuban Missile Crisis — October 1962
Nothing else convincingly takes first place.
On October 14, 1962, an American U-2 reconnaissance aircraft photographed Soviet medium- and intermediate-range ballistic missile installations being constructed in Cuba. Washington suddenly faced nuclear-capable Soviet forces barely 90 miles from Florida.
For nearly two weeks, President John F Kennedy and Soviet leader Nikita Khrushchev attempted to control a confrontation in which military action by either side could have triggered retaliation from the other.
The most dangerous point came on October 27 — later remembered as Black Saturday.
An American U-2 was shot down over Cuba. Another U-2 accidentally wandered into Soviet airspace near Siberia. American commanders were considering strikes against Cuba, while Kennedy resisted military pressure for an attack and invasion. Contemporary US military discussions explicitly recognised that escalation could become nuclear.
Then came perhaps the most terrifying incident of all.
Deep beneath the Atlantic, the Soviet submarine B-59 was being pursued by US forces trying to force it to surface. The Americans were using signalling depth charges, but officers aboard the submarine did not know whether war had already begun.
B-59 was carrying a nuclear torpedo.
Evidence assembled from Soviet participants indicates that commander Valentin Savitsky became convinced his submarine might be under attack and ordered the nuclear torpedo prepared. Brigade chief of staff Vasily Arkhipov helped calm the situation and recognised that the Americans were signalling rather than beginning combat. The weapon was never fired.
Had a nuclear torpedo destroyed an American carrier group during the most dangerous day of the Cuban Missile Crisis, nobody can know whether Kennedy could have prevented retaliation.
That combination — deployed nuclear weapons, active military confrontation, a U-2 already shot down, invasion plans under discussion and a nuclear-armed submarine commander believing he was under attack — makes October 1962 the strongest candidate for the closest humanity has ever come to nuclear war.
2. Able Archer 83 — November 1983
The frightening feature of Able Archer was that one side was practising nuclear war while the other feared it might not be practising at all.
Relations between Washington and Moscow were already extremely hostile in 1983. Soviet intelligence had been ordered to watch obsessively for signs that NATO might be preparing a surprise nuclear first strike.
Then NATO conducted Able Archer 83, an exercise rehearsing the procedures through which an escalating European conflict could move towards nuclear weapons release.
Declassified Soviet and Western material subsequently showed that Soviet leaders and military personnel viewed the exercise with unusual alarm. Soviet air units, including nuclear-capable forces, increased readiness, and later intelligence assessments concluded that the danger of misunderstanding had been genuine.
One crucial decision may have helped prevent the situation becoming worse. Lieutenant General Leonard Perroots, the US Air Force intelligence chief in Europe, did not recommend escalating Western readiness in response to the unusual Soviet preparations.
That mattered because an American escalation could itself have appeared to confirm Soviet suspicions.
There is no evidence that Moscow was seconds from pressing a nuclear launch button, and exaggerated versions of Able Archer should be avoided. But the deeper danger was systemic: NATO did not fully appreciate how frightened Soviet leaders had become, while Soviet leaders were looking at a military exercise through a worldview in which exercises could disguise preparations for attack.
Two nuclear superpowers were effectively interpreting each other's defensive behaviour as potentially offensive.
That is how deterrence can become a trap.
3. Stanislav Petrov and the Five Phantom Missiles — September 1983
Only weeks before Able Archer, Soviet lieutenant colonel Stanislav Petrov encountered a different road towards catastrophe.
On September 26, 1983, Petrov was duty officer at the Soviet Oko early-warning command centre when the system reported that an American intercontinental ballistic missile had been launched.
Then it indicated more launches.
Five missiles appeared to be heading towards the Soviet Union.
Petrov faced an extraordinary decision under intense time pressure. Rather than automatically accept the computer's warning, he reasoned that an American first strike would almost certainly involve far more than five missiles.
He classified the warning as false. He was right.
Petrov did not personally possess a button capable of instantly launching Soviet missiles, and accounts claiming that one man literally had unilateral control over whether the world ended oversimplify Soviet command procedures.
But his judgement mattered enormously because erroneous warning information entering the command chain during one of the Cold War's most hostile periods could have generated further escalation.
The episode exposed an enduring weakness in nuclear deterrence: ultimately, even extraordinarily sophisticated systems depend on imperfect machines being interpreted by imperfect humans with only minutes available.
In this case, human scepticism defeated machine certainty.
4. The Norwegian Rocket Incident — January 1995
The Cold War was supposedly over.
That did not stop Russia from briefly wondering whether a nuclear attack was underway.
On January 25, 1995, Norwegian and American scientists launched a Black Brant XII research rocket to study the aurora borealis. Russia had been notified about the planned launch, but the information did not successfully reach the personnel operating its warning system.
Russian radars detected the rocket and its trajectory appeared potentially consistent with that of a submarine-launched ballistic missile.
Russia's warning apparatus swung into action.
President Boris Yeltsin, Defence Minister Pavel Grachev and Chief of the General Staff Mikhail Kolesnikov were connected through Russia's nuclear command system. The incident is widely regarded as the first known occasion on which a Russian president activated the Cheget nuclear command briefcase in response to a potential attack warning.
Russian forces tracked the rocket while officials tried to determine whether they were observing an attack.
Eventually its trajectory made clear that Russia was not the target.
The alarm ended.
The significance of the episode is enormous. Moscow and Washington were no longer ideological enemies on the verge of superpower confrontation. Yet the machinery designed for nuclear retaliation remained capable of transforming an innocent scientific experiment into a national command emergency.
Nuclear danger did not require hatred.
It required uncertainty.
5. The Yom Kippur War Superpower Alert — October 1973
The 1973 Arab-Israeli war became dramatically more dangerous when it threatened to pull the United States and Soviet Union directly into the conflict.
As Israeli forces gained the advantage and surrounded Egypt's Third Army, Soviet leader Leonid Brezhnev proposed that American and Soviet forces intervene to enforce the ceasefire. His message warned that Moscow might otherwise consider unilateral action.
Washington responded by placing American forces worldwide on heightened alert, including nuclear forces.
The US State Department's historical account describes the episode as bringing the United States closer to nuclear confrontation with the Soviet Union than at any point since Cuba.
American forces were moved to DEFCON 3, the highest state of peacetime military alert. Strategic Air Command and other nuclear-associated forces were among those affected.
The crisis ultimately subsided and Soviet forces did not intervene directly.
But it demonstrated another mechanism through which a nuclear war could begin: not because Washington or Moscow actually wanted to destroy each other, but because both became entangled on opposing sides of somebody else's regional war.
That mechanism remains relevant today.
6. The Sino-Soviet Nuclear Crisis — 1969
The United States and Soviet Union were not the only nuclear powers capable of approaching catastrophe during the Cold War.
In 1969, fighting erupted along the border between the Soviet Union and Mao Zedong's China. What began as territorial clashes became serious enough for American intelligence analysts to examine whether a major Sino-Soviet war could erupt.
Even more alarmingly, Washington received indications that Moscow was exploring the possibility of attacking China's nuclear facilities.
A September 1969 memorandum to President Richard Nixon recorded Soviet approaches probing how the United States might react to such an attack. An American intelligence estimate concluded that while a Soviet strike was not considered likely, there was some chance Moscow could convince itself that Chinese nuclear and missile facilities could be attacked without triggering prolonged war.
Eventually diplomacy prevailed.
But 1969 matters because it shattered the simplistic assumption that nuclear danger existed only between Washington and Moscow.
Two communist nuclear powers nearly became military enemies — a reminder that ideology does not guarantee strategic restraint and that relationships between nuclear states can deteriorate extraordinarily quickly.
7. The NORAD Training-Tape False Alarm — November 1979
At 8:50am on November 9, 1979, American warning systems appeared to show something horrifying: a massive Soviet missile attack.
US command centres reacted.
Missile crews were alerted, command aircraft became involved and officials rushed to determine whether the United States was facing the beginning of nuclear war.
It wasn't.
A simulated attack scenario used for training had mistakenly entered the operational warning system. Official documentation later confirmed that the bogus attack information was automatically transmitted to major command centres before being identified as false.
Checks worked. Satellite and radar evidence failed to confirm the supposed Soviet attack, and the warning was dismissed within minutes.
That is reassuring — up to a point.
The terrifying lesson is that a piece of training data managed to masquerade as the opening of World War Three inside the machinery responsible for detecting precisely such an event.
8. India and Pakistan's Nuclear Confrontations
India and Pakistan have created perhaps the world's most persistent regional nuclear flashpoint.
Both demonstrated nuclear weapons in 1998. The following year they fought the Kargil conflict over Kashmir, proving that the possession of nuclear arsenals did not eliminate conventional war between them.
The danger has never completely gone away.
The underlying problem is geography, disputed territory, terrorism, short missile flight times and military doctrines that can force leaders to make decisions quickly. Historically, Pakistan has retained the possibility of first nuclear use while India has relied primarily on retaliatory deterrence.
The warning became even more relevant after the India-Pakistan conflict of May 2025. SIPRI says attacks reached Pakistani air and missile bases likely to have nuclear-related roles, while both sides nevertheless took measures to prevent further escalation. Disinformation also complicated assessment of what was actually happening.
That conflict did not necessarily come closer to nuclear use than Cuba, Able Archer or Petrov.
But it revealed why South Asia remains one of the places where the next nuclear crisis could emerge extraordinarily quickly.
What Could Trigger the Next Nuclear War?
The most probable route to nuclear war is unlikely to resemble a leader waking up one morning and deciding to destroy the planet.
History suggests something more dangerous: escalation.
A conventional war begins. One side believes it is losing. Command systems are attacked. Intelligence becomes unreliable. Leaders fear their nuclear forces could soon be destroyed. Warning systems detect something ambiguous. Decision time collapses.
Then deterrence stops being stable.
These are the biggest future trigger scenarios.
1. Russia and NATO Fight Directly
This remains arguably the most dangerous route to a genuinely global nuclear exchange because Russia and the United States possess the overwhelming majority of the world's nuclear weapons.
The war in Ukraine has already created repeated situations in which Russian and NATO personnel, aircraft, drones, missiles and intelligence systems operate in overlapping strategic environments.
Russia has also revised its nuclear doctrine and repeatedly used nuclear signalling as part of its confrontation with the West. NATO planners increasingly have to consider how a conventional European conflict could operate beneath an explicit nuclear threat.
The nightmare scenario is not necessarily a planned Russian nuclear attack on London or Washington.
It could begin with something smaller: NATO personnel killed by a strike, a Russian aircraft shot down, a major attack on a NATO state, a conventional NATO-Russia clash in the Baltic region or an attack Moscow interprets as threatening its strategic command infrastructure.
Once NATO and Russian forces were openly killing each other, the escalation ladder would become dangerously steep.
2. China Invades or Blockades Taiwan
A war over Taiwan could place the United States and China in direct large-scale combat for the first time.
That alone would transform the international system.
Both possess nuclear weapons. Both depend heavily on sophisticated satellites, command networks, radar, cyber infrastructure and long-range conventional missiles. During a conventional war, attacks intended to blind or disable the enemy could unintentionally threaten systems connected to nuclear command and warning.
That creates what strategists call entanglement: a conventional attack can appear strategically nuclear even when no nuclear weapon has been fired.
Recent assessments have consequently identified a Taiwan crisis as one of the clearest pathways towards US-China nuclear escalation. China's arsenal is also expanding rapidly, adding another dimension to future deterrence calculations.
The danger would become acute if either side believed defeat threatened fundamental national interests.
3. Another India-Pakistan War Escalates Too Fast
This may be the most plausible regional nuclear crisis.
Both countries already fight periodic confrontations. Both possess nuclear weapons. Their major population centres and military targets are comparatively close together, compressing warning and decision times.
The 2025 confrontation demonstrated how rapidly terrorism, retaliatory strikes, military bases, missiles, drones and information warfare can become entangled.
SIPRI specifically warned that strikes against nuclear-related military infrastructure combined with third-party disinformation risked converting conventional hostilities into a nuclear crisis.
A future terrorist attack blamed on Pakistan-based militants could restart that chain with even less restraint.
4. North Korea Miscalculates
North Korea presents a different problem.
Its leadership wants nuclear weapons partly because it believes they guarantee regime survival. But that creates a dangerous incentive during a major conventional conflict: Pyongyang could fear that waiting too long might allow its nuclear arsenal or leadership structure to be destroyed.
North Korea continued developing new nuclear-capable delivery systems throughout 2025. SIPRI estimated at the start of 2026 that Pyongyang may have assembled around 60 warheads while possessing fissile material for at least 30 additional weapons.
A missile test mistaken for an attack, a major clash with South Korea, an attempted decapitation strike or the apparent beginning of invasion could therefore produce extreme pressure on North Korean decision-making.
5. A Middle East War Crosses the Nuclear Threshold
The Middle East contains an unusual combination of nuclear ambiguity, missile warfare, proxy networks and unresolved proliferation.
Israel is widely assessed to possess nuclear weapons despite maintaining deliberate ambiguity. Iran has not been established as possessing a nuclear weapon, but its nuclear programme remains one of the world's central proliferation disputes.
The 2025 attacks by Israel and the United States on Iranian nuclear facilities created a new precedent: military force was directly employed against nuclear infrastructure during a regional war. SIPRI subsequently warned that the conflict damaged the broader non-proliferation environment and ended continuous international monitoring of Iranian nuclear sites.
If Iran eventually decided to construct nuclear weapons, the region could become dramatically more unstable. An Israeli-Iranian war conducted between two nuclear-armed states would create an entirely different strategic environment.
6. A False Alarm During a Real Crisis
This may be the most underestimated threat of all.
Petrov, NORAD and the Norwegian rocket demonstrate that false alarms are not hypothetical.
The real nightmare would be a false warning occurring not during relative peace but during an active Russia-NATO, US-China or India-Pakistan war.
Imagine satellites already being attacked, cyber networks failing, leaders expecting incoming missiles and warning systems suddenly reporting launches.
The historical presumption that decision-makers can calmly investigate would become far weaker.
The more frightened leaders are, the more believable the false alarm becomes.
7. Cyber, AI or Space Warfare Blinds Nuclear Command Systems
Modern technology has created capabilities Cold War planners never had to confront at today's scale.
Cyber operations can interfere with communications. Anti-satellite weapons could damage sensors. Artificial intelligence can accelerate intelligence assessment while also producing convincing misinformation. Conventional and nuclear systems increasingly share infrastructure.
SIPRI warned in 2026 that warfare across land, sea, air, cyber, space and information domains is generating new escalation pathways because nuclear and non-nuclear systems increasingly interact. Its research also notes growing AI integration into nuclear-related systems even as China and the United States publicly endorse retaining human control over decisions to use nuclear weapons.
A cyberattack does not need to launch a nuclear missile to create catastrophe.
It merely needs to convince somebody that one has been launched — or that their ability to retaliate is disappearing.
The Nuclear Paradox
Nuclear weapons have probably helped prevent direct war between some major powers because the consequences are so overwhelming.
Yet the same mechanism produces the paradox at the centre of nuclear deterrence.
For deterrence to work, every nuclear state must persuade its opponents that under certain circumstances it really would use weapons capable of killing millions.
That means missiles remain ready. Submarines patrol. Bombers train. Exercises rehearse nuclear procedures. Warning systems watch continuously for attacks that may allow leaders only minutes to react.
And the number of actors is growing more complicated.
At the start of 2026, all nine nuclear-armed states were modernising or strengthening their arsenals. Arms-control structures have weakened, transparency has declined, direct strategic communication between some nuclear powers is inadequate and thousands of warheads remain operationally available.
The historical record therefore delivers a disturbing conclusion.
Humanity survived the Cuban Missile Crisis partly because Kennedy and Khrushchev ultimately chose compromise. It survived B-59 because Arkhipov helped stop panic becoming action. It survived the 1983 Soviet false alarm because Petrov distrusted his computer. It survived Able Archer because Western forces did not answer Soviet nervousness with another escalation. It survived the Norwegian rocket because Russian leaders waited for more information.
Different crisis. Same pattern.
Nuclear deterrence is supposed to prevent catastrophe through certainty: certainty that aggression will be answered and certainty that nuclear war cannot be won.
History shows that the machinery surrounding it is actually filled with uncertainty — incomplete intelligence, human judgement, technical failure, misread intentions and decisions made under severe time pressure.
The greatest danger may therefore never be a leader deliberately ordering World War Three.
It may be two leaders who each believe the other one already has.
